The theory of steel slag fall off from Oxygen Lance:

Some certain steel slag is accumulated from blasts on the taper lance segment. After the lance is lifted, clearance between the steel slag and lance appear, and then cracks will appear between parts of the steel slag and the lance after the steel slag has been cooled by the circulating water. Parts or all of the steel slag sticking to the taper lance segment will fall off from the lance due to the steel slag being expanded because of sudden heating and due to repeated washing by the liquid and steel slag
